"Pants" vein jump graft for portal vein and superior mesenteric vein thrombosis in transplantation of the liver.

چکیده
PORTAL VEIN TlIROMBOSIS (PVT) or thrombosis of its tributaries has been a formidable surgical challenge in orthotopic hepatic transplantation (OHT). At one time, PVT was considered a relative contraindication for OHT. However, refinements of the technique to reconstruct the portal vein after thrombosis have included venous grafting procedures (1, 2). The technique of the venous "jump" graft from the superior mesenteric vein (SMV) for PVT has been standardized and should be considered in cases of PVT to establish portal flow to the hepatic allograft (1-5). One of the limiting factors of the venous jump-graft technique is the lack of a sufficient segment of SMV from which the venous graft can be anastomosed. Herein, we report a technique of anastomosing two iliac venous allografts separately to two jejunal branches of the SMV to reconstitute portal vein flow to the hepatic allograft in a patient with both portal vein and SMV thrombosis.

Liver Transplantation in the Presence of Portal Vein Thrombosis
Thrombosis of the portal vein with or without patency of its tributaries used to be a contraindication to orthotopic liver transplantation (OLTX) until quite recently. Rapid progress in the surgical technique of OLTX in the last few years has demonstrated that most patients with portal vein thrombosis can be safely and successfully transplanted. Presented here is a series of thirty-four patient...

Severe portal vein thrombosis (PVT) is often considered a relative contraindication for living donor liver transplantation due to high associated risks and morbidity. Meanwhile, improvement in operative techniques, resulting in higher success rates has removed PVT from the list of contraindications in deceased donor liver transplantation (DDLT). In this report, we describe a surgical technique ...

BACKGROUND Liver transplantation (LT) in patients with portal vein thrombosis (PVT) remains a challenge for transplant surgeons. In this study, we included a group of patients with PVT who underwent LT, and analyzed patient outcomes. METHODS A total of 356 patients who underwent LT consisting of 167 cases of deceased donor LT and 189 cases of live donor LT at Chang Gung Memorial Hospital Link...

Portal vein thrombosis (PVT) is a rare complication of liver transplantation which can lead to graft failure and patient death. Treatment can be difficult, especially in cases of PVT from the intrahepatic portal vein to the proximal jejunal veins. A 55-year-old woman had undergone living-donor liver transplantation with splenectomy for end-stage liver cirrhosis due to hepatitis C with hepatocel...
